Hospice Comfort Care Team Specialized Services are Tailored to Fit Your Needs
PERSONAL PHYSICIANS continue to actively participate in the patient's care including writing orders, and consulting with the hospice team.
MEDICAL DIRECTOR/HOSPICE PHYSICIANS offer their expertise in pain and symptom management and oversee the patient’s hospice care.
NURSES manage patient's symptoms, provide education to patients, caregivers and coordinate care with the rest of the hospice care team.
CHAPLAINS offer spiritual care in accordance with wishes of the patient/family, often coordinating with your clergy or religious leader.
SOCIALWORKERS conduct assessment, provide counseling, education and other skilled interventions to enhance patient and family coping.
CERTIFIED NURSING ASSISTANTS/Home Health Aides & Homemaker provide personal compassionate treatments such as bathing, nail and skin care, and grooming. Prepare meals, change bed lines, straighten room, laundry and more!
BEREAVEMENT COORDINATORS offer grief support and counseling to patients, families, and caregivers. Services are offered throughout the duration of hospice care and are followed by 13 months of bereavement support.
TRAINED VOLUNTEERS provide companionship and a calming presence to the patient. They may also offer short periods of respite to the caregiver to allow them to run an errand or keep an appointment.
