About the Role

The PCC Nurse will spend most of their time in the office assisting the other hospice team members but will get to spend some time in the field visiting patients as well. A PCC nurse can be either an Registered Nurse (RN) or Nurse Practitioner (NP) and based on their credentials will also perform the following:

RN - Manage a case load of a couple patients providing direct care in the field.
NP - Perform face-to-face encounters and work closely with Medical Director on coordination of care
Skills & Qualifications

Strong customer service skills
Effective verbal and written communication skills
Works well with other team members
Highly organized with the ability to solve problems
Ability to work in office and field settings
Experience with computers and software
Licensed as an RN or NP in State of WI

The hospice patient care coordinator (PCC) nurse in an essential member of the hospice team. The PCC nurse assists the team members in the field so they can stay focused on providing the best care . The PCC nurse provides assistance to the team in many ways including:

  • Coordinating care with other medical professionals

  • Writing orders

  • Sending faxes and emails

  • Receiving and making phone calls

  • Providing detailed documentation

  • Auditing charts

  • Ordering DME and Medications

  • Assist hospice team to stay focused on providing care

  • Attend IDT meetings

Job Types: Full-time, Part-time

Salary: $40.00 - $60.00 per hour

Expected hours: 30 – 40 per week
